In positions of higher responsibility Petty Officer Hack has had difficulty working with teams and leading subordinates on-duty, and has exhibited questionable judgement off-duty. He has been placed TAD in a non-classified environment.
38-TEAMWORK. Despite high technical knowledge he was removed from a pilot qualification and training program due to his argumentative approach with instructors. His removal from the program resulted in a 20% loss in instructor availability, loss of three weeks of instructor time, and hindered progress in achieving flag level directives.
34/35/37/39-LEADERSHIP. As he gained knowledge and rose to senior watch positions he refused to follow command operating and training procedures. He was cited for poor leadership by his peers and subordinates, and described as disrespectful by supervisors. His disruptive behavior resulted in his removal from watchstanding.
36-MILITARY BEARING. Exhibiting questionable judgement off-duty he socialized with anti-government protestors resulting in the loss of confidence to safe guard classified material and revocation of his security clearance.
Since removal of his military duties Petty Officer Hack has contributed by volunteering extensively with the local Japanese community and taking higher education courses.

MEMBER STATEMENT ICO 2014NOV16 TO 2015NOV15 EVALUATION
The outburst with my instructor was regrettable as I admired his background and role as a subject matter expert. However, I had difficulty in completing the assignment in the time frame he set as well as the quality of information he wanted. My requests for more time and more guidance were denied and I was frustrated. By the end of the day, we ended up shouting at each other. I take full responsibility for losing my military bearing.
When I met with senior leadership after the incident, I realized that I was being seen as rebellious and pretentious. In order to remedy this, I started looking at how I interacted with people in general. In order to better interact with people, I started to look into psychology and management studies.
Unfortunately, I was in a car accident caused by a person without insurance. Dealing with this as well as trying to stay in good standing after causing an incident at the command was taxing. I started to mistake jokes or normal criticism as attacks. Not wanting to end up in an unpleasant workplace, I started asserting myself more but also kept myself distracted by focusing on work assignments.
Despite my efforts to stay within command instructions and policies, in combination with a mistaken, negative bias towards communications, I acted in ways that were adverse as well as disrespectful. I regret the negative interactions I have had with individuals in my command, and am currently endeavoring to correct the way I interact with both my superiors and those I am supervising. As well, I will conduct myself in ways that show better alignment with command instructions and policies.
As to the allegation of association with Japanese protesters, I did happened upon some individuals next to a sign in Japanese, in a remote area of Okinawa. They told me about the forest they were trying to protect from the construction of a new base. Although I listened to them, I was just trying not to appear rude by just ignoring them and leaving. I can see now that I made the wrong decision and should have immediately left once I found out they were protesters.
I realize that I have found myself in unusual situations with regards to foreign nationals. I acknowledge that multiple series of poor choices led me to those situations, but none of those choices were ever made with any intentions not in line with my oath to serve the United States. In the future, I will better evaluate my surroundings and avoid situations which give the impression of impropriety.
It has been a rough year, but I have been doing what I can to still be a productive and better person. It is my desire that the actions I have taken to improve myself will better reflect my dedication to duty and, furthermore, my ability to work with and lead others.
